OBJECTIVE: To determine the effectiveness of increasing the dietary content of soluble fibre (psyllium) or insoluble fibre (bran) in patients with irritable bowel syndrome. DESIGN: Randomised controlled trial. SETTING: General practice. PARTICIPANTS: 275 patients aged 18-65 years with irritable bowel syndrome. INTERVENTIONS: 12 weeks of treatment with 10 g psyllium (n=85), 10 g bran (n=97), or 10 g placebo (rice flour) (n=93). MAIN OUTCOME MEASURES: The primary end point was adequate symptom relief during at least two weeks in the previous month, analysed after one, two, and three months of treatment to assess both short term and sustained effectiveness. Secondary end points included irritable bowel syndrome symptom severity score, severity of abdominal pain, and irritable bowel syndrome quality of life scale. RESULTS: The proportion of responders was significantly greater in the psyllium group than in the placebo group during the first month (57% v 35%; relative risk 1.60, 95% confidence interval 1.13 to 2.26) and the second month of treatment (59% v 41%; 1.44, 1.02 to 2.06). Bran was more effective than placebo during the third month of treatment only (57% v 32%; 1.70, 1.12 to 2.57), but this was not statistically significant in the worst case analysis (1.45, 0.97 to 2.16). After three months of treatment, symptom severity in the psyllium group was reduced by 90 points, compared with 49 points in the placebo group (P=0.03) and 58 points in the bran group (P=0.61 versus placebo). No differences were found with respect to quality of life. Fifty four (64%) of the patients allocated to psyllium, 54 (56%) in the bran group, and 56 (60%) in the placebo group completed the three month treatment period. Early dropout was most common in the bran group; the main reason was that the symptoms of irritable bowel syndrome worsened. CONCLUSIONS: Psyllium offers benefits in patients with irritable bowel syndrome in primary care..

Background/Objectives:Skeletal muscle plays important role in the regulation of whole-body metabolism. In skeletal muscle, uptakes of glucose and fatty acid from circulation are facilitated by transmembrane substrate transporters GLUT4 and FAT/CD36, respectively. The aim of this study was to determine the effect of dietary glycemic index (GI) on GLUT4 and FAT/CD36 gene expressions in human skeletal muscle after a single bout of exercise.Subjects/Methods:Eight male subjects completed a 60-min cycling exercise at 75% maximal oxygen consumption (VO(2 max)), and were immediately fed an isocaloric meal containing either high-GI (HGI) or low-GI (LGI) diets, with similar proportions of carbohydrate, fat and protein in a crossover design. Muscle samples from deep vastus lateralis were taken by needle biopsy immediately after exercise and 3 h after exercise. Results:After exercise, the HGI diet produced significantly greater glucose and insulin responses compared with the LGI diet, as indicated by the greater area under the curves. Both diets resulted in rapid reductions in plasma fatty acid and glycerol below fasting level. GLUT4 mRNA was downregulated by both HGI and LGI diets to a comparable extent, whereas GLUT4 protein levels were not changed during this short period. FAT/CD36 mRNA and protein levels were substantially decreased with the HGI diet below baseline, but not with the LGI diet. Conclusion:This study found a significant dietary GI effect on post-exercise FAT/CD36 gene expression in human skeletal muscle. This result implicates that the differences in dietary GI are sufficient to alter fat metabolism.European Journal of Clinical Nutrition advance online publication, 26 August 2009; doi:10.1038/ejcn.2009.100.

LCHF:are som vanligt förståndigt folk! ;-)

Effect of bite size and oral processing time of a semisolid food on satiation
Background: Food texture plays an important role in food intake regulation. In previous studies we showed a clear effect of viscosity on ad libitum food intake and found indications that eating rate, bite size, and oral processing time (OPT) could play a role.
Objective: The objective was to determine the effect of bite size and OPT of a food on satiation, defined as ad libitum food intake.
Design: Twenty-two healthy subjects participated in all 7 test conditions. Bite sizes were free or fixed to small bite sizes ({approx}5 g) or large bite sizes ({approx}15 g). OPT was free (only in combination with free bite size) or fixed to 3 or 9 s. Subjects consumed chocolate custard through a tube, which was connected to a peristaltic pump. Sound signals indicated OPT duration.
Results: Subjects consumed significantly more when bite sizes were large than when they were small (bite size effect: P < 0.0001) and when OPT was 3 s rather than 9 s (OPT effect: P = 0.008). Under small bite size conditions, mean (±SD) ad libitum intakes were 382 ± 197 g (3-s OPT) and 313 ± 170 g (9-s OPT). Under large bite size conditions, ad libitum intakes were much higher: 476 ± 176 (3-s OPT) and 432 ± 163 (9-s OPT) g. Intakes during the free bite size conditions were 462 ± 211 g (free OPT), 455 ± 197 g (3-s OPT), and 443 ± 202 g (9-s OPT).
Conclusion: This study shows that greater oral sensory exposure to a product, by eating with small bite sizes rather than with large bite sizes and increasing OPT, significantly decreases food intake.

Kommentarer
